I could vaccinate my child at 12 months and again at 4 years of age.  

I could decide to discuss vaccination further with my doctor or health care provider before making a decision.

I could decide to discuss vaccination further with my partner and family before making a decision.

I could delay vaccination until my child is older.

I could decide against vaccination.
